http://www.gravityforms.com/ does some nifty stuff - it’s not free but the really nice components for any CMS never are
Having used Gravity Forms for some out of the box things like creating pdf’s that get populated with form data and emailed automagically so the info can be printed for internal use to full user integration with Infusionsoft* for variable content display and marketing - this thing can do it all starting at just 39 bucks per year.
*Crazy complicated integrations require a more expensive version - but chances are you don’t have any REALLY nutso to contend with - so the cheap one should do way more than you need…
